All new clients, please print, read, and complete the following forms.  Adults, please complete the Adult Intake Information form. Parents of adolescents seeking treatment, please complete the Adolescent Intake Information form for your child.

 

If you are unable to do so, please arrive 15 minutes early to your session in order to fill them out in-person.
